Warranty Request Submission

If you are having an issue with a Morimoto assembly (headlights, tail lights, grille lights, or 3rd brake light) please select the assembly product type.

For all other components, make sure to select the non-assembly option.


For our support team to help with this issue, we’ll need some information from you to be included on this form.

  • If you purchased directly from us, we’ll need to know
  • Your order number
  • Contact information (name, email address originally ordered with, and phone number)
  • An accurate shipping address should any replacements be due
  • The SKU or part number
  • A brief description of the issue with supporting images/video (10mb limit).
  • If you purchased from an authorized dealer, we’ll need the same information, but also proof of purchase from that dealer (a copy of the receipt or order confirmation email is sufficient).
